What the study measured
İz and Dindaroğlu compared panoramic radiographs from 150 people in three groups: no congenital tooth agenesis, bilateral agenesis, and unilateral agenesis. They measured condylar height, ramus height, and mandibular length, then computed standard asymmetry indices using the Habets formula.
No group differed significantly in the raw linear measurements. The asymmetry indices told a different story. The unilateral agenesis group showed a ramus asymmetry index of 2.05 and a mandibular asymmetry index of 2.15, against 0.89 and 0.82 in controls and 1.50 and 1.40 in bilateral cases, differences the authors report as statistically significant (p < 0.001). The effect was larger when the missing teeth were posterior (condylar asymmetry 7.67 versus 3.98) or in the mandible rather than the maxilla.
What this is, and what it is not
This is observational human data: a single-center, cross-sectional comparison of radiographs. On the tier ladder used here (see method) it sits in T3 territory, small and narrow, and it is associational. It cannot show that the missing teeth caused the asymmetry; both could follow from whatever disrupted development on that side. It also says nothing about treatment of any kind.
Its relevance to this publication is indirect but real. The Phase II program for the anti-USAG-1 antibody targets children with congenital agenesis. Studies like this one define what that population actually looks like clinically: not simply people missing teeth, but people whose jaw development may already be measurably asymmetric by the time a therapy could reach them. If a future trial claims restored teeth, the craniofacial context in papers like this is part of what “success” would have to mean.
Limits worth keeping in view
Panoramic radiography is a screening tool, not a 3D measurement; the authors themselves reserve CBCT for detailed evaluation. Asymmetry indices computed from 2D projections are sensitive to head positioning. And 150 participants across three groups leaves small cells once cases are split by jaw and tooth position, which is where the most striking numbers appear.
Where we differ from the coverage
Agenesis is usually framed as a gap to be filled. This study is a reminder that the condition is developmental, not just cosmetic: the record shows measurable skeletal associations, not merely missing teeth. Any therapy aimed at this population will be judged against that fuller picture.
